This page is for families and students who have gone through the Intent to Register and selection process, and have received notification of their acceptance at Mountainville Academy.

Congratulations on your acceptance to Mountainville Academy!  At Mountainville Academy, we truly believe in our motto of “Building New Traditions of Excellence”, and we look forward to including your student and family in our community of builders.

 

We are pleased to provide you with important information about finalizing the registration of your student, including a list of items that are necessary to complete registration as well as a completed registration deadline.  If at any time you are unsure about the process or requirements, communication is vitally important to a successful outcome and we encourage you to contact us with any questions you may have. 

 

In order to register your new student at Mountainville Academy, you will need to provide the following:

 

 Original Birth Certificate – The state requires that we see the original birth certificate, then make a copy for the student’s file. Please make sure your student’s birth certificate is accessible, or if necessary, make arrangements to obtain one as soon as possible.  For students born in Utah County, you can visit www.co.utah.ut.us and click on “Online Service” for information on how to apply for a new birth certificate.

 Current Immunization Record – The law requires that all students be current on their immunizations or have an exemption granted by the Utah Department of Health.  Parents of incoming 7th grade students in particular should be aware that there may be additional immunization requirements for their student.  Please visit http://www.immunize-utah.org/provider/school/schoolchild_requirements.htm to ensure your student is up-to-date with the most recent requirements.

 Proof of Vision Screening (Kindergarten only)

 Special Education Information if you have it (if applicable) – If your student has been serviced in Special Education we will need to obtain a current copy of the IEP or 504 so that our resource team can address the needs of your student.  (Mountainville Academy will request records from your student’s current school upon registration.)

 Copy of your student’s most recent report card if you have it (1st through 8th grades - Mountainville Academy will request records from your student’s current school upon registration.)

 Proof of Custody/Guardianship (if applicable) – Only a parent or guardian with “physical custody” can register a student.  In the case of divorce, a copy of the divorce agreement, signed by a judge, is required to establish physical and custodial rights.  If the student to be registered is not living with the custodial parent or guardian, please arrange to meet with the Administrative Director of the school immediately.

 Copy of withdraw form, if possible, from the school student was most recently registered – Students may only be registered in one school at a time.  Mountainville Academy cooperates with district and charter schools to ensure that students are not dually enrolled.

 Completed Registration Forms – The necessary registration forms are available below as well as at the school office.  If possible, please take time to complete the forms at home to minimize the waiting time for other registrants at the school. 

 

If any of these items are missing at the time of registration, we will not be able to complete the registration process.  If we do not receive your completed registration packet by the given deadline and alternative arrangements have not been made, the status of your student will be changed from “accepted” to “declined”, meaning you have forfeited registration for your student, and the position will be offered to the next waiting student.  Again, if for any reason you are unable to meet this deadline, please contact us to make alternative arrangements.

Please note that any student who has been expelled from any school, including Mountainville Academy, during the preceding 12 months may be denied enrollment. (Utah State Code 53A-11-904(3)).

Important Note: Please print your registration forms single-sided.  Forms will be separated and distributed to various departments for processing.



Fee Waivers: The 2010-2011 Income Eligibility Guidelines are not available at this time.  Individuals may apply for fee waivers based on the 2009-2010 Income Eligibility Guidelines with an understanding that determination of fee waiver eligibility will be made based on the 2010-2011 Income Eligibility Guidelines when the updated information becomes available.

Parents wishing to apply for fee waivers must have also completed a Declaration of Household Income form (included in the registration packet above).
